The twist is shocking because the film cheats to achieve it. If I was a kid when I watched this I probably would have been blown away by the big reveal at the end, but even as an adult I still thought the ending was a fun twist.

But I must admit I enjoyed this movie far more then I thought I would, and it holds up really well. Not many 80's films can make that claim.

It's been years since I've seen this movie, but I do remember renting it thinking it was going to be terrible and then being amazed at how good it was. The twist in the movie is pretty shocking. While it is a shame that the movie isn't more well known, your observations in the last paragraph are spot on about how difficult it is to sell it.
